A. General Communication

  • Secure communication via TherapyPortal is preferred.

  • By using email, you consent to responses by email unless you request another communication method. Text messaging requires the separate, optional consent below.

  • Providers do not accept social media requests to protect your confidentiality.

  • Therapy sessions may be supported by HIPAA-compliant transcription tools (e.g., Freed AI or Twofold Health). You may opt out at any time without penalty.

B. Optional Text Messaging (SMS/MMS): Consent and Terms

Message types: Leaf Psychiatry, LLC offers optional text messages about appointments, scheduling changes, billing or account notifications, reminders to complete forms or review secure portal messages, and responses to nonurgent administrative inquiries. This consent does not authorize advertising or promotional text messages.

How to opt in: Enter your mobile number, select “I agree” below, and sign or electronically submit this form. Providing a phone number or signing the general treatment agreement alone does not authorize text messages. Texting is optional and is not a condition of receiving treatment or purchasing services.

Frequency and charges: Message frequency varies based on your appointments, requests, and communication with our office. Messages may be recurring and may be sent using automated technology. Message and data rates may apply.

Opting out: Reply STOP to opt out of this text-messaging program at any time. You may also contact our office to withdraw consent. You may receive one final text confirming your request; further messages from this program will stop unless you opt in again. Declining or withdrawing SMS consent will not affect your eligibility for care.

Privacy: Our Privacy Policy is available at https://www.leafpsychiatry.com/privacy-policy. We do not sell or share mobile numbers or SMS consent information with third parties or affiliates for their own marketing or promotional use. Information may be provided to vendors only as needed to deliver messages on our behalf, subject to confidentiality protections. These SMS terms are included in Section 5 of our general consent at https://www.leafpsychiatry.com/policies.

Security and limitations: Ordinary text messages are not end-to-end encrypted. Messages may be seen by others with access to your phone and may reveal your relationship with Leaf Psychiatry. Use TherapyPortal for sensitive health information. Texts are not monitored continuously and must not be used for emergencies. Call 911 for emergencies or 988 for crisis support. Wireless carriers are not responsible for delayed or undelivered messages. This consent does not waive your privacy rights or replace our Notice of Privacy Practices.
